Rehabilitation of element 37 with implant and connective tissue graft: a clinical case report and interdisciplinary approach

Rehabilitation of posterior regions with dental implants faces unique challenges due to insufficient gingival tissue, especially in element 37 (lower second molar), an area poorly documented in the literature. Connective tissue grafting (CTG) is crucial for tissue stability, but its application in lower molars is underexplored. To report a clinical case of rehabilitation of element 37 with an implant and CTG, highlighting the interdisciplinary approach between implantology and periodontics. A 45-year-old male patient with missing element 37 and thin gingival biotype. An implant (Cone Morse Titamax 3.75 × 11.5 mm) was placed, followed by a 3-month osseointegration period. Subsequently, a subepithelial CTG (15 × 8 mm) was harvested from the hard palate and placed on the buccal aspect of the implant. At 6 months, gingival tissue showed increased thickness (>2 mm), pink color, harmonious emergence profile, and absence of complications (implant exposure or peri-implantitis). The prosthetic rehabilitation restored masticatory function and aesthetics. The combined protocol (implant + CTG) proved feasible and effective for the rehabilitation of element 37, reinforcing the importance of an interdisciplinary approach. CTG was essential for tissue stability in a high-load bearing region, providing a model for poorly documented posterior areas.
