Fixed prosthetic rehabilitation of atrophic maxilla using all-on-four technique and digital surgical guide: a case report

Abstract

Computer-guided techniques have revolutionized the rehabilitation of atrophic maxillae, with guided surgery enhancing precision. This study aimed to report a clinical case of total atrophic maxilla rehabilitation using integrated digital planning, surgical guide fabrication, and the all-on-four technique. In a 54-year-old patient, bone access to the maxilla was achieved via a transmucosal approach without flap elevation, with four implants placed at a final torque of 45N. Insignificant tissue injury was observed, confirmed by postoperative gingival integrity, precise implant positioning (verified through periapical radiographs), and favorable progression with mild edema and discomfort. The protocol demonstrated technical feasibility for complex rehabilitations, providing excellent outcomes, reduced tissue trauma, decreased surgical time, appropriate implant placement, and enhanced rehabilitative predictability.
